The Centers for Medicare & Medicaid Services (CMS), previously known as the Health Care Financing Administration (HCFA), is a federal agency within the United States Department of Health and Human Services (DHHS) that administers the Medicare program and works in partnership with state governments to administer Medicaid, the State Children's Health Insurance Program (SCHIP), and health insurance portability standards.

Hospital-Acquired Condition Reduction Program

In October 2014, CMS began reducing Medicare payments for subsection (d) hospitals that rank in the worst-performing quartile with respect to hospital-acquired conditions (HACs). Hospitals with a Total HAC Score above the 75th percentile of the Total HAC Score distribution may be subject to the payment reduction. This table contains hospital’s measure scores and Total HAC scores. The measure scores comprise the Total HAC Score.
